The Telemedicine Dilemma

You are Dr. Citra, the island's sole physician. You are in the capital city for a conference and have obtained permission from the director to use telemedicine during the conference, as no other doctor is available to cover your shift that day. It is Sunday morning, and the other medical staff who are not on duty cannot be contacted. Your emergency department nurse, Nurse Tono, contacts you via a secure video teleconference link. The patient, Mr. Budi, a 68-year-old retired fisherman from the remote island of Pulau Telo, and his son, Ari, are with the nurse at the clinic.


Nurse's Initial Report (via video):
"Doc, good morning. Sorry to interrupt. I have Mr. Budi here. It is his face—it started drooping on the right side this morning. You know his history... I'm very worried it's another stroke."
