It is Sunday morning, and the other medical staff who are not on duty cannot be contacted. Your emergency department nurse, Nurse Tono, contacts you via a secure video teleconference link. The patient, Mr. Budi, a 68-year-old retired fisherman from the remote island of Pulau Telo, and his son, Ari, are with the nurse at the clinic.

"Doc, good morning. Sorry to interrupt. I have Mr. Budi here. It is his face—it started drooping on the right side this morning. You know his history... I'm very worried it's another stroke."
