You are Dr. Citra, the island's sole physician. It is Sunday morning, and the other medical staff who are not on duty cannot be contacted. Your emergency department nurse, Nurse Tono, contacts you via a secure video teleconference link. The patient, Mr. Budi, a 68-year-old retired fisherman from the remote island of Pulau Telo, and his son, Ari, are with the nurse at the clinic.
