Epilogue

At 3‑month follow‑up, Mr. Green reported improved exertional capacity, with no orthopnoea or paroxysmal nocturnal dyspnoea. Weight decreased by 3.5 kg, blood pressure averaged 124/74 mmHg, and there were no heart failure admissions. Investigations showed HbA1c 7.0%, eGFR 57 mL/min/1.73 m2, while echocardiography showed a LVEF of 40%.

Mr. Green is advised that the focus remains on adherence, lifestyle, and close monitoring. Mr. Green thanks you and remains optimistic, especially since he now has his granddaughter to spoil.
