You prescribe this angiotensin converting enzyme inhibitor for Mr. Hill. Mr. Hill is finally feeling more optimistic and feels that this new medication will be right for him.

Two weeks later, he presents to his GP complaining that his blood pressure remains elevated. The GP looks over to you and asks why you have chosen to prescribe this drug for Mr. Hill, who is expected to have low renin due to his age and ethnic background. ACEIs are very beneficial in hypertensive patients, particularly in patients with type 2 diabetes mellitus or chronic kidney disease, but they are not the best option for Mr. Hill.
