You are Dr. Eka, the Dokter PTT (Pegawai Tidak Tetap - Temporary Contract Doctor) serving the Puskesmas (Community Health Center) of Pulau Tello. This island is part of the Nias Selatan Regency, Sumatra, Indonesia. The geographic context is critical: the facility is isolated by the open ocean. The nearest referral hospital, in Gunungsitoli, is a 4- to 6-hour boat ride away, and the seas are currently treacherous due to the monsoon season.

Facility Capabilities:

• Diagnostics: Hematology analyzer (often needs calibration), basic microscopy (malaria), rapid diagnostic tests (NS1, Malaria), X-ray (analog).
• Therapeutics: Basic pharmacy (IV fluids, diazepam, paracetamol, antibiotics), oxygen concentrators.
• Staff: Nurse Rina, a senior nurse with two decades of local experience. She acts as the cultural broker between the "modern" doctor and the traditional community.

Upon entering the UGD, the sensory overload is immediate. On the single treatment bed lies Bayu, a 15-year-old male. He is physically restrained by two elderly family members—his Grandfather (Pak Darman) and Grandmother (Ibu Siti). Bayu is screaming, his voice hoarse and guttural. He thrashes against the restraints, eyes wide with terror, fixated on empty corners of the room.

"Jangan! Jangan mendekat! Ada api! Setan merah!" (Don't! Don't come closer! There is fire! Red demon!).
