Follow up visit after ER

After being released from the hospital, the patient visits you at the office. 

He says that he did not even realize how things have gotten so bad with him ending up in the hospital. He mixed up the pain meds, at some point using the maximum of everything just because he was in a lot of pain. 

You realized that you need to go over with him everything, once more, write everything down and even spend some time with him explaining even more about pain meds, their adverse effects etc.
