Basal insulin can control hyperglycaemia but has no heart failure outcome benefit, it may also cause weight gain, and is not preferred for heart failure risk reduction. The increased complexity of treatment (e.g. injections, blood glucose monitoring) and cost also make insulin less favourable unless absolutely needed. Insulin is reserved for severe hyperglycaemia or when other agents are inadequate. This is not the best drug for Mr. Green at this point. Please reconsider which other drug can manage type 2 diabetes but also help with weight loss and reduce his risk for cardiovascular complications. Please go back and try again. |
