Methionine

You prescribe methionine. An hour later, you are paged to see Andrew. The nurse informs you that Andrew seems to have slipped into a coma. The medical team is unable to do anything further for Andrew. You think it is best that you personally inform his mother of her son's death.  

You reflect on the events of the evening and realize that methionine is an oral substitute for N-acetyl cysteine. This drug may also be used to replenish glutathione levels, if N-acetyl cysteine is not available. You realize that since you administered N acetyl cysteine, there was no reason to prescribe this drug and this may have contributed to Andrew's death.
