N acetyl cysteine

Based on Andrew's severe presentation, you think that ingestion of paracetamol occured at least two days ago. 

You remember that NAC exerts its maximum protective effects up to 8 hours after ingestion. However, you decide to administer NAC, as you remember that the paracetamol antidote should still be prescribed, even at this potentially late stage.

Andrew appears to be disoriented, confused and drowsy. You are particularly concerned about his mental status and level of consciousness. Based on the West Haven criteria, you deem that Andrew is showing signs of hepatic encephalopathy. You are considering pharmacological treatment.
