Dr. Demetriou commends you on your excellent advice, as she is aware that antacids may affect the absorption of other medications. Dr. Demetriou explains to Mr. Mason that his recent exacerbation is most likely due to reduced absorption of his heart failure medication due to magnesium hydroxide. She advises Mr. Mason to let her know how he does, when he takes his antacids within two hours of his heart failure medication. 

Dr. Demetriou also refers Mr. Mason for an endoscopy and cultures to help identify the cause of his GI discomfort. 

Endoscopy

An upper endoscopy shows a duodenal ulcer. Cultures of the lesion grow spiral-shaped, motile, oxidase-positive, catalase-positive rods with multiple flagella at one pole. The doctor decides that pharmacotherapy is indicated.
