It may be used in the treatment of pulmonary embolism but due to its unpredictability in effect, it is generally reserved for patients who are hemodynamically unstable in anticipation of a potential need for thrombolysis or emboloctomy, where reversal of heparin action (with protamine) would be required to reduce the risk of bleeding.
