Taking Medical History

understand if he had history of kidney stones,  known hematological diseases, infections, if he had undergone urogenitals invasive procedures etc.

Ask what drugs he takes (Nebivolol, Hydrochlorothiazide, Metformin, Apixaban,Tamsulosin, Amiodaron, LevoTiroxin) 

Ask  him if he's an active smoker, or if he used to smoke a lot. (Yes, he is still an active smoker)
