•Patient is pale, sweaty and tachypneic but consious and able to speak
 
•Vital signs
 
•HR 115bpm
 
•BP 85/65mmHg
 
•SpO2 90% on room air, recovered to 95% after administration of 2l/min O2
 
•RR 28/min
 
•No fever
 
 
•Left calf mild edema and pain
 
•S1S2: loud, rhythmic, normal S1, accentuated P2, and right side S3 gallop
 
•Lungs: Lungs clear to auscultation and percussion, no rales/rhonchi/wheezes

 
•ECG (XIII)
 
•Chest (XIV)
 
•ABGs: PH 7.3, PCO2 26mmHg, PO2 136mmHg, HCO3- 8mmol/l, cLac 3.3mmol/l
 

Current condition

  • Syncope after getting out of bed
  • Approximal duration 2-3min
  • Recovered alone – stand straight  after the event
  • Pleuritic pain of acute onset
  • Reports left calf pain since 2 days
