Diagnostic Synthesis:

Severe Dengue with Expanded Dengue Syndrome (EDS).

• Component 1: Dengue Shock Syndrome (Pulse pressure 20mmHg, Hct 52%).
• Component 2: Dengue Encephalopathy (GCS 9, Psychosis).
• Note: Without CSF analysis or MRI, distinguishing Encephalitis (viral invasion) from Encephalopathy (edema/metabolic) is impossible. However, the management in this setting is identical: Supportive Care directed at perfusion.
