The absence of prior psychiatric history and the acute onset coincident with the illness excludes primary schizophrenia. The specific visual hallucinations (shadows/demons) are common in organic deliriums, including Dengue Encephalopathy, where cerebral edema or cytokine storms disrupt cortical function.
