The Dialogue: Guided Physical Examination

Doctor: "Mr. Hartono, for this next part of your physical exam, I’m going to have Nurse Rina perform the checks while I guide her. It helps our team make sure we are all seeing the exact same things. We'll be talking through the steps out loud so you know exactly what we're looking for. Are you comfortable with that?"



Mr. Hartono: "Yes, Doctor, that is fine with me."

Doctor: "Thank you. Alright, Nurse Rina, let’s start with the eyes to check for any jaundice or signs of anemia. Go ahead and walk Mr. Hartono through it."

Nurse Rina: "Excuse me, Mr. Hartono. I’m just going to gently pull down your lower eyelids to look at the color of your eyes. Could you look straight up at the ceiling for me?"

(Nurse Rina performs the check)

Nurse Rina: "Doctor, the sclera are clear, and the conjunctiva look pink and healthy—no signs of anemia."

Doctor: "Excellent observation. That’s very reassuring, Mr. Hartono. Next, Nurse Rina, let’s check his neurological reflexes, specifically looking for asterixis, or a flapping tremor."

Nurse Rina: "Okay. Mr. Hartono, could you please hold both your arms straight out in front of you, and bend your hands back at the wrist, like you are pushing against a wall? Hold it right there for a few seconds."

(Patient holds the position; Nurse Rina observes)

Nurse Rina: "He is fully alert and oriented, holding steady. No tremor observed."

Doctor Budi: "Spot on. Your reflexes and nerve control are perfect, Mr. Hartono. Finally, let's move to the abdomen. Nurse Rina, keeping in mind that Mr. Hartono has been feeling some tenderness in his upper abdomen, how should we approach this?"

Nurse Rina: "I should make sure my hands are warm, and I will start pressing gently on the lower part of the abdomen first, moving towards the tender area last so we don't cause unnecessary pain."

Doctor Budi: "Exactly right. Let's proceed. Mr. Hartono, Nurse Rina is going to press lightly on your stomach now. You are in control here—if anything feels too uncomfortable, you just say 'stop' and we will immediately."

(Nurse Rina palpates the abdomen carefully)

Nurse Rina: "Are you doing okay, Mr. Hartono? ... Doctor, I feel the tenderness in the upper quadrant as he mentioned, but I don't feel any enlargement of the liver or spleen."

Doctor Budi: "Great technique, Nurse Rina. And thank you, Mr. Hartono, you did very well. It's good news that those organs aren't swollen. You can relax your arms now, and we'll get your gown adjusted."
