•      History of TB: Rapid pulmonary Test TB (+) 4 weeks ago. Started anti tuberculosis drug 4-FDC (RHZE) daily.
•      History of Hypertension (HTN): Diagnosed 10 years ago. Controlled with Amlodipine 10 mg (once daily).
•      History of Type 2 Diabetes Mellitus (T2DM): Diagnosed 8 years ago. Controlled with Metformin 500 mg (three times daily).
•      History of Alcohol Use: "I've never drunk alcohol, Doc. Never smoked either."
•      History of Liver Disease: "I've never diagnosed with liver disease."
•      History of Allergies: No known drug allergies.
