Simvastatin (Statin)

Simvastatin has no clinically meaningful haemodynamic interaction with sildenafil and does not cause acute vasodilation that would explain severe hypotension. The statin’s main concerns are myopathy risk at higher exposures and interactions via CYP3A4, which do not relate to this acute episode. This choice does not account for Mr. Garcia’s sudden collapse.
