Jacob Hill, a 65-year-old man of Afro-Caribbean descent, presents to the Emergency Department in severe respiratory distress. He is too unwell to provide any information. His wife informs you that he began complaining about flushing and itching about half an hour ago. This was followed by throat tightness, wheezing, and dyspnoea. His wife called an ambulance.

She explains that he was recently diagnosed with hypertension and started on new medication. She cannot remember the name of the drug. He has no other significant medical history.

You ask about allergies and Mrs. Hill reports that her husband is allergic to a pain killer. She shows you the box: celecoxib. You suspect that anaphylaxis was caused by cross-reactivity of the new anti-hypertensive medication with celecoxib.
